AMA AND OTHERS CAUTIONS “RETAIL CLNICS; BIG BAD WOLF!”

Let’s start with some of the arguments these groups put forward as reasons for concern in using a Retail Clinic!

  • Ø Do not have access to all chart records.
  • Ø Causes Fragmentation of care by adding another provider.
  • Ø Staffed often by Physician Assistances or Nurse Practitioner
  • Ø No patient follow up care.
  • Ø Chronically ill are more complicated than clinics usually see.  (Note it doesn’t say can’t handle)
  • Ø Question if have patient’s best interests at heart!

I looked in detail at five articles about these warnings.  The first covers the period around 2007.  Titled AMA Goes After Walmart Style Retail Clinics, it warns of the same concerns.  “Our primary focus is patient safety and patient care, and the retail clinics have a different mission of selling products and prescriptions,” said Dr. Rodney Osborn.  The AMA stance at that time was to lobby for increases regulations of these Retail Clinics.  Typically doctors often site regulations and dealing with insurance as an over regulated problem.

The other articles cover and aim at those clinics owned by CVS, Walgreen’s, Target and other groups. On Feb. 23, 2014 (my 52 birthday updated a statement disapproving of the Retail Clinics.  Again the list above covers their reasons for concern.

An article written in Aug. of 2012 shows these statistics.

  • Ø Total visits go from 1.5 million (2007) to 6 million (2009)
  • Ø The same study notes that there are 557 million visits to Doctors and 177 to ER’s
  • Ø One significant change in demographics was that those over 65 were at 7.5% (2000 to 2006) and at 14.7 (2007 to 2009).
  • Ø About 40% of the visits from 2007 to 2009 were for immunizations.

I now have a few questions and statements I think need to be considered.

  • Ø How often can the people that visited a Retail Clinic did so because of lack of access to an appointment needed now?
  • Ø How many ER’s have access to a patient’s health record or chart?
  • Ø How many Pediatricians really know a patient and remember that child’s chart when called after hours?
  • Ø How many Nurse Practitioners or PA’s are employed by Pediatricians or Doctors?
  • Ø Is it really fair to say no access to follow up care?  I say this because many say to follow up with your family practitioner or come back to them if they do not have one!
  • Ø If it is still true that most(almost half, 40%) go for immunizations, is this really a big issue?
  • Ø What study said this are that?  Compare it to what is actually going on.
  • Ø Should Pharmacists really be giving vaccinations when they are bogged down just filling prescriptions?
  • Ø How many groups are going to have a formal relationship with your Pediatrician?
  • Ø Is some of the disjoint and lack of communication the fault of the AMA for going after the Retail Clinics instead of really working with these groups to shore up the holes? Or, is this just about the bottom line?
  • Ø How many people complain about not enough time with their doctor?

I would think again about what the AMA and AAP says about these clinics.  I would think also about getting prescriptions where pharmacists are doing a lot of vaccinations.  I am a pharmacist and will get plenty of feedback for this I am sure.

The bottom line is go to places that you feel comfortable.  I can personally vouch for a Nurse Practitioner owned clinic.   Excellent care is what my husband (also a pharmacist), my son and I get when we go there.  This does not stop me from having a family practice physician.  I have gone to this man since 1987 and will not go elsewhere  besides that NP owned clinic in Yukon, OK.  They do not compete but compliment each other.

The Failed Republic!!!

I couldn't hardly believe what I had heard today. Cut spending on our military when we most need them. Cut troop personnal by 120,000 troops. 120,000 more people unemployed and on welfare and food stamps. Is this anyway to treat our soldiers. No! And cut their pay for those who aren't let go. They don't make enough now. If anyone deserves Cadillac plans on Insurance, pay, and benefits its our fighting men and women. Sure as heck not Congress or the Adminstration. The number one job of the Federal Govt. is to protect our Country and its People. And they can't even do that. They can spend Trillions on failed Banks, Corporations, Failed Energy Companies, Acorn, Planned Parenthood, Fast and Furious, Benghzia Scandal, Shutting down Coal Companies, etc., etc., But can't take care of the Nations Safety or our Troops?  They must need to divert more money for the Gigantic Welfare State that they are putting us into. Or Obamacare. The militaries budget is only $500 Billion. Thats a drop in the bucket compared to all the Trillions that have been wasted under this Adminstration and Congress.The security to our Nation and whole free world is in the balance here. They'll fund our enemies , and countries that don't like us all day long. They'll even fund terrorist groups. They now want to pull funding from our ( The Peoples) one ally. The Military. With the full help of the Republican Congress they just gave them blank check debt ceiling where there is no ceiling. So they can waste alot more trillions we dont' have. Then try to convince us we need to cut the military. Hog Wash. Theres something that stinks like a skunk here.None of this makes any sense. Unless your anti-military, anti- american. And your trying to deliberately make us more unsafe and take America down. And if you cut the real military you can sink more money into your private police state. And what really ripps me, Is the gutless Republicans in Congress will probably go along with this. Mind numbingly, Stupid.

ORIGIN OF GOD BLESS AMERICA

This is worth the time, I had no idea there was a prelude.




Do you remember Kate Smith?

Frank Sinatra considered Kate Smith the best singer of her time, and said that when he and a million other guys first heard her sing "God Bless America" on the radio, they all pretended to have dust in their eyes as they wiped away a tear or two.

Here are the facts...

The link at the bottom will take you to a video showing the very first public singing of "GOD BLESS AMERICA". But before you watch it, you should also know the story behind the first public showing of the song.


The time was 1940. America was still in a terrible economic depression. Hitler was taking over Europe and Americans were afraid we'd have to go to war. It was a time of hardship and worry for most Americans.

This was the era just before TV, when radio shows were HUGE, and American families sat around their radios in the evenings, listening to their favorite entertainers, and no entertainer of that era was bigger than Kate Smith.

Kate was also large; plus size, as we now say, and the popular phrase still used today is in deference to her, "It ain't over till the fat lady sings". Kate Smith might not have made it big in the age of TV, but with her voice coming over the radio, she was the biggest star of her time.

Kate was also patriotic. It hurt her to see Americans so depressed and afraid of what the next day would bring. She had hope for America, and faith in her fellow Americans. She wanted to do something to cheer them up, so she went to the famous American song-writer, Irving Berlin (who also wrote "White Christmas") and asked him to write a song that would make Americans feel good again about their country. When she described what she was looking for, he said he had just the song for her.

He went to his files and found a song that he had written, but never published, 22 years before - way back in 1917. He gave it to and she worked on it with her studio orchestra. She and Irving Berlin were not sure how the song would be received by the public, but both agreed they would not take any profits from God Bless America. Any profits would go to the Boy Scouts of America. Over the years, the Boy Scouts have received millions of dollars in royalties from this song.


This video starts out with Kate Smith coming into the radio studio with the orchestra and an audience. She introduces the new song for the very first time, and starts singing. After the first couple verses, with her voice in the background still singing, scenes are shown from the 1940 movie, "You're In The Army Now." At the 4:20 mark of the video you see a young actor in the movie, sitting in an office, reading a paper; it's Ronald Reagan.

To this day, God Bless America stirs our patriotic feelings and pride in our country. Back in 1940, when Kate Smith went looking for a song to raise the spirits of her fellow Americans, I doubt whether she realized just how successful the results would be for her fellow Americans during those years of hardship and worry..... and for many generations of Americans to follow. Now that you know the story of the song, I hope you'll enjoy it and treasure it even more.

New Orleans Latest ( Near ) Carjacking

A hysterical woman ( a city Facilities Maintenance worker ) was almost car jacked at 2am Saturday morning, in my neighborhood.
 
This latest of many assaults takes advantage of public safety resources being focused on New Orleans parade season and upcoming Mardi Gras.
 
The combatant stepped from the neutral ground ( localese for median ), waved a gun & demanded her car.
 
The would-be victim hit reverse, went down the block until she hit a parked van and exited screaming.
 
My room mate got her inside our house & called 911.
 
Not that it matters but both the suspect and intended target were Black.
 
What does matter is that not one so-called local "snivel" rights activist will hit the streets on behalf of a wife who had just finished working a parade clean up and didn't deserve to be frightened within an inch of her life by one of the low lifes I call, " chocolate klansmen. "
 
When persons, parents, preachers & politicians fail; when police are under staffed & overwhelmed, the last, the only line of defense is the public, unless they're a bunch of sheep hoping against hope the wolves won't eat them next.

Less than two months before taxes are due, House Republicans will pass a slate of bills aimed at protecting taxpayers from Internal Revenue Service abuses.

The House GOP also plans to move legislation that would force the government to reveal more clearly how taxpayer money is being spent.

Republicans' legislative plans follow a year of constant scrutiny of the IRS, after the tax collection agency admitted it improperly targeted for extra scrutiny conservative groups applying for tax exempt status, delaying their approval.

Many Republicans said that act was an overtly political attempt to prevent these groups from having an impact in the 2012 election.

Rep. Peter Roskam (R-Ill.) is the author of two of the bills to be considered next week, both of which respond to the targeting scandal.

"Practically every day, there's news of a new scandal at the IRS, and unfortunately it seems the administration is incapable of getting the IRS under control and restoring faith in the agency," Roskam said last year when he proposed the legislation. "The targeting of individuals by the IRS based on their political and social beliefs cuts to the core of American's trust in government, and it's time to institute reforms in order to protect taxpayers from further abuse."

One of his bills is the Taxpayer Transparency and Efficient Audit Act, H.R. 2530. This bill would require the IRS to tell taxpayers when it shares their tax information with another government agency, and limits the time people can be subjected to an IRS audit to one year.

Republicans are wary the IRS will improperly share personal tax information with other agencies as it tries to implement ObamaCare and make determinations about who might qualify for tax credits when buying health insurance.

Another bill from Roskam up next week is the Protecting Taxpayers from Intrusive IRS Requests Act, H.R. 2531. This bill would prevent the IRS from asking about people's religious or political beliefs.

Both of Roskam's bills will be called up under a suspension of House rules, which means they'll need a two-thirds majority vote to pass.

GOP leaders are expected to call up another IRS bill from House Ways & Means Committee Chairman Dave Camp (R-Mich.). Camp's bill, which will be called up under a rule and can pass with a simple majority, would delay IRS rules that would limit the political activities of certain tax-exempt groups.

The House will also look at two other suspension bills meant to ensure taxpayers know how their money, once collected by the IRS, is being spent.

The first of these is the Taxpayers Right-to-Know Act, H.R. 1423. This bill from Rep. James Lankford (R-Okla.) requires all federal agencies to describe all programs under their agency, their costs, the number of employees running each program, and possible duplication. Reports would be due each year.

Agencies would also have to publish performance reviews, improper payment rates and other information.

Based on this information, the Office of Management and Budget would have to publish an annual report that identifies overlapping programs, and recommendations on how to reduce this duplication.

The second transparency bill is the Taxpayer Transparency Act, H.R. 3308. This bill from Rep. Billy Long (R-Mo.) requires executive branch agencies to disclose when advertisements are paid for at taxpayer expense.

The bill is a reaction to the Department of Health and Human Service's 2013 effort to advertise ObamaCare. But Long said the bill would also require the government to tell people when it's behind ads for food stamps or other government services.

"The Taxpayer Transparency Act requires government agencies to put a disclaimer at the bottom of any advertisement that is paid for by taxpayers," Long said last fall. "By calling attention to how taxpayer dollars are spent, my bill will serve as a reminder that all services provided by the government are ultimately paid for by the taxpayers."

Respectfully Ma'am - more than 130 million of us have been demanding this for a couple of years now.....
WE do not see this as a lack of citizen support - we see this as an abject failure of Congress as a body to observe and abide by Federal Law and the Constitution of the United States.

In our millions we have shut down your phone systems on multiple occasions, marched on DC unlike any time in the history of this nation - millions of Americans have brought DC to a screeching halt and gridlock several times!!!!
Did Congress pay attention?
No, they did not.

You say in your article you want citizen support.... yet citizens in their millions HAVE lawfully petitioned their government for redress - and Congress ignores us.
Ma'am - we have all grown weary petitioning members of Congress because we see NO action - despite the evidence and testimony Congress already has in it's possession. Congressman Bob Goodlatte received testimony a few months ago telling Congress that if the government, this administration - refused to abide by the Rule of Law - why should citizens abide by the Rule of Law?
What happened?
NOTHING!
And you will note - Citizens continue to abide by the Rule of Law....

My personal opinion is that if Congress followed Federal Law and the Constitution - Lois Learner and Erick Holder would have been immediately arrested - but what happened?
NOTHING.
We also believe that if Congress would abide by Federal Law and the Constitution - and begin arresting and charging these people with treason - you would get all the evidence you need to begin arresting ALL of them....
Because none of these people are going to be willing to do Life Sentences - or the death penalty - for mr obama.... they would start running their mouth and all the truth would come out in volumes......

We all want to know when Congress is going to arrest this man and every person in his administration for TREASON. If Congress fails in this - millions of Americans are going to die. The apparent belief in Congress that all of this "is just politics" is a FALSE belief................ Anyone in Congress who adheres to this false belief is either complicit in his crimes - or deceiving themselves.......

His first targets will be Veterans with any Operations experience - we are the most dangerous citizens to him.... then he will move against every other Military Veteran, then he will target the average citizen who has spoken out against him.
We just do not know when he's going to do it.
Many of us believe we are teetering on the verge of a civil war - and those of us with Combat experience do not want to see that, no - not at all...

Yet we also see what we are faced with - and we fail to understand how Congress could not see what is right in front of your face!

He has already raised an unconstitutional "Army".
He has provided this "Army" with US Military weapons, billions of rounds of ammunition (and continues to buy more), Armored Military Combat vehicles, to include DRONES!
He gave himself the ability by executive fiat to declare Martial Law - in a time of peace...
He has given himself the ability to murder or indefinitely incarcerate any American without evidence OR Due Process of Law - and Congress signed off on it!!!!!!!!!!!!!!!!!!!!!!!

You (Congress) are treating each and every illegal act as an isolated incident - when you should take the tactical view and see the entire "Big Picture" of what he has done and what he is doing......

CNN Fires Piers Morgan, What Took Them So Long? 

CNN announced today they are cancelling at long last "Piers Morgan Live".. The prime-time show which has had what could be best described as lousy to rotten ratings, for good reason, is at long last been cut from the faltering network's evening lineup.

Morgan who came to the US under a cloud after he published a series of faked pictures depicting Iraqi prisoners being abused by British soldiers in the UK Tabloid "The Daily Mirror". Morgan had also been implicated in the infamous British Phone hacking scandal and was questioned for his role in the fiasco. This should have been a heads up to anyone even pondering the hiring of this guy.

The problem is that many people don't even know who Saul Alinsky is, let alone that Obama is an ardent follower-- even teaching a course in and employing his "rules for radicals" as a community organizer. (Hillary did her thesis on it). Alinsky's key rule is -- " in war the end justifies almost any means". Another key Alinsky rule (in his book Rules for Radicals) is-- " ridicule is man's most potent weapon-- pick the target, freeze it, personalize it, and polarize it".
(I.e. The Tea Party). This information is extremely important.

There are 8 levels of control that must be obtained before you are able to create a social state. The 1st is the most important.

1. Healthcare

– Control Healthcare and you control the people.

2. Poverty –

Increase the Poverty level as high as possible, poor people are easier to control and will not fight back if you are providing everything for them to live.

3. Debt-

Increase the debt to an unsustainable level that way you are able to increase taxes and this will produce more Poverty.

4. Gun Control-

Remove the ability to defend their self from the Government, that way you are able to create a Police State.

5. Welfare-

Take control of every aspect of their life’s (Food, Housing, and Income).

6. Education-

Take control of what the people read and listen too – take control of what the children learn in school.

7. Religions-

Remove the belief in God in Government and schools.

8. Class Warfare-

Divide the people in the wealthy and the poor, this will cause more discontent and it will be easier to take (Tax) the wealthy with the support of the poor.

Wireless cars to cause grief

Wireless cars are in our very near future. Wireless cars will be able to invade your privacy.  As a women I feel that they are a threat to my safety.  Wireless cars can give access to where you are going, your speed and other private information to other cars.  Wireless cars we make us susceptible to abduction and harrassment. This is a very real serious concern for women and children. We need to make congress aware that wireless cars are a security threat.
